Dear Parent,

The Ohio Department of Health requires that all 7th graders have a booster of Tdap(preferred) or TD.

If your child has had this immunization over the summer months please send me a copy from the doctor’s office or health department.  If your child has not yet received this booster you need to make arrangements to have it before September 14 so that your child will not be excluded from attending school after that date.  Attached is a flyer advertising a walk-in clinic at the Fairfield  County Department of Health.
